Estimated Fees

General fees-$125/hr. prorated based on nature of the appointment, distance of travel, complexity of problem/duration of the visit, or telephone/text/email consult time.  Estimated fees can be assessed upon request prior to your visit.

*Please note that a standard $25 fee is applied to all visits; this covers administrative fees and operating expenses associated with treatment. This fee cannot be billed to insurance and will be owed by the patient at the time the final invoice is issued.

Insurance

We are currently accepting Aetna, United Healthcare, Coventry, Meritain, Tricare, GEHA and UMR insurance for direct billing.  New insurances being added all the time!  Please contact us see if your insurance is billable or for information on self-pay.

Don’t see your insurance listed here? We can see you but payment is due at the time of service on the card used to book the visit. We can provide you with a medically coded invoice to present to your insurance for possible reimbursement upon request! Most insurances cover lactation support, but may not contract with or accept direct billing from a lactation consultant. Lactation support is covered by the Affordable Care Act, it’s the law! Financial Assistance

Momentum Lactation recognizes that there are sometimes financial circumstances that make it difficult to afford the cost of lactation care. Insurance coverage for lactation care often varies greatly between carriers requiring patients who need care to pay out-of-pocket. We do not want this to become a barrier to families seeking lactation or postpartum support. We offer a sliding fee scale based on household size and income level, and encourage families to complete a brief financial assessment to determine their eligibility for sliding scale billing.
